TEAM  REGISTRATION  FORMS ~

You may use these forms when registering your players for a tournament. Please fill them out as completely and as accurately as possible.  The information you write on these forms will be used to make pairings, etc.

Remember to use a separate page for each section (i.e. K-3; 4-6, etc.).
